Skilled Nursing Services

At Complete Home Health Care our services are led by Registered Nurses who are dedicated to our patients’ individual health care needs. All care is coordinated and directly communicated with their physician and provided in a home, apartment or assisted living facility.

Post Surgical Care

We understand that additional post-surgical care, beyond a normal hospital stay, is one of the main reasons our patients require care. Complete Home Health Care provides in-home health care to patients recovering from:

  • Heart Surgery (Open Heart, By-Pass, etc)
  • Hip or Knee Replacement Surgery
  • Auto or Accidental Injuries Surgeries
  • Wound care
  • All Other Post-Surgery Care


Physical Therapy

Complete Home Health Care provides rehabilitation services to help our patients work towards regaining muscle strength, joint movement, independent ambulation and mobility. Their skilled therapist will develop a treatment plan to assist in the recovery
of physical disabilities, injuries, and disease processes.  

Services Include:

  • Home Exercise Programs
  • Pain Relief Therapies
  • Balance, Gait Training, Assistive Devices
  • Muscle Re-education, Endurance
  • Post-Operative Programs
  • Post-CVA Therapy


Occupational Therapy

Our Occupational Therapists help our patients return and adapt to a routine of daily living. They will also learn to live more fully with any temporary and/or permanent disability and become skilled at new ways to adapt to their home environment. Goals may include techniques for sensory integration, energy conservation, work simplification, and joint protection.

Services include:

  • Restoring self-care skills in feeding, dressing, bathing, grooming, personal hygiene, toileting
  • Assess safety conditions in the home
  • Provide for adaptive equipment
  • Improve fine motor coordination
  • Provide patient, family, and caregiver education


Home Health Aide

We provide Home Health Aide services if our patient requires assistance with personal care while being treated at home by a Registered Nurse, Physical or Occupational Therapist and/or Speech Language Pathologist. These services are provided once it is determined by your physician and nurse there is a need for care.

The major function and role of a Home Health Aide is to assist a patient with personal care and limited household tasks.

Services include:

  • Helping the patient bathe and dress
  • Assisting with mouth and hair care
  • Assist patient with transfers in and out of bed
  • Assist and support exercise training
  • Assist in the support and training of self-care techniques
  • Changing linens
  • Light cleaning, straightening of patient care area
  • Preparing or assisting patient with light breakfast or lunch, as time permits


Social Worker

Social Workers' roles are to provide a means to improve one's quality of life.

Services include:

  • Consultation with patients and their families in coordinating community resources, financial assistance, and family counseling
  • Assistance in case management and linking patients with agencies and programs that will help them with their psychosocial needs



Registered Dietician

A Complete Home Health Care Registered Dietician can help manage a health condition or provide dietary consultation to patients and their families. A dietician will confer with other health care professionals and develop an individual plan to meet specific nutritional requirements.

Services provided:

  • Dietary Consultations for heart conditions, diabetes, obesity, celiac disease, and irritable bowel syndrome, etc.
  • Assistance in creating meal plans that can improve a medical condition or deficiency
  • Advise of potential risks and benefits of special foods
  • Help plan recipes and food substitutions so food choices are not limited
  • Provide culturally sensitive nutritional assessments
  • Provide dietary resource materials
